Entry Permits And Cancelled Entry Permits For Confined Space Entry

Once an employee has to enter a confined space on any type of job, they will require an entry permit, which must be signed by the entry supervisor. It must also be posted at all the entrances of the area, or made available to all employees before they enter the area.

 

Here are the items that the entry permits should include:

  • The name of the confined space to be entered.
  • The names of the authorized entrant or entrants.
  • The names of the eligible attendants.
  • The names of the persons in charge of entry supervision.
  • The test results.
  • The tester’s signature or initials.
  • The name and signature of the supervisor who authorized entry.
  • The purpose of the entry.
  • The known hazards of the confided space.
  • The measures which must be taken to isolate permit spaces and to control/eliminate hazards.
  • The name and contact numbers for rescue and emergency service, as well as the means of contacting them.
  • The date and authorized duration of entry.
  • The acceptable entry conditions.
  • The communication procedures and equipment to maintain contact during entry.
  • Any additional permits, which may have been issued authorizing work in the confined space.
  • The special procedures, equipment and other information which is necessary for employee safety.

 

In relation to cancelled entry permits, the entry supervisor must cancel the entry permits after the assignment is finished or if a new condition arises. If there are any new conditions, they must be documented on the cancelled permit and used in revising the permit space.
